1. Group leaders are responsible for paying their sitters and then submitting this form for reimbursement. Whether sitters are paid on a weekly or monthly basis can be determined at the group leader’s discretion. 

2. Maximum reimbursement per community group per week is $20. If your group meets longer than 2 hours or needs more than one sitter, any additional payment to the sitter(s) will need to be provided by the group. To determine rates for individual sitters, please use the chart below.

3. You may request reimbursement for up to five events per form. Please be sure to enter the info for each event in the space below.

4. This form must be submitted to the church office within thirty days of the last event recorded.

5. Checks will be mailed within two weeks after the church office receives your completed form.
