1. Group leaders are responsible for paying their sitters and then submitting this form for reimbursement. Whether sitters are paid on a weekly or monthly basis can be determined at the group leader’s discretion. 

2. The childcare reimbursement rate is $20 per hour per childcare worker, up to two hours. If there are eight children or more, a second childcare worker must be hired. Groups may also be reimbursed for a second childcare worker when there are fewer than eight children, if approved by Church Administration. If your group meets longer than two hours, any additional payment to the sitter(s) will need to be provided by the group. 

3. You may request reimbursement for up to five events per form. Please be sure to enter the info for each event in the space below.

4. This form must be submitted to the church office within thirty days of the last event recorded.

5. Checks will be mailed within two weeks after the church office receives your completed form.
